UNUM Life and AD&D
All employees are provided with Life and AD&D insurance equal to 1 x their Salary, up to a maximum benefit of $500,000. However the Guaranteed Issue maximum is $300,000, therefore you will be automatically enrolled into this plan for 1x your salary, up to the $300,000 maximum guarantee issue. Those that qualify for more than the guarantee issue amount you must provide satisfactory Evidence of Insurability by completing and submitting the Evidence of Insurability form located on the left toolbar. Please follow instructions on the from in order to submit the form directly to UNUM. After approval your Life and AD&D coverage amount will be adjusted to 1 x your salary up to the $500,000 maximum.

UNUM Short & Long Term Disability
You will be automatically enrolled into both the Short and Long Term Coverage with UNUM. There is no action required on your part.
Short Term Disability: Is 60% of salary up to a $2,500 weekly maximum benefit. Benefits are paid after a 7 Day Elimination period and paid up to a 12 Week maximum benefit period.
Long Term Disability: Class 1 (Equity Partners) Benefits are 60% of salary up to a $20,000 maximum benefit per month. Benefits are paid after a 90 day Elimination Period and are payable until your Social Security Retirement Age.
Class 2 (Partners, Associates, COO): Benefits are 60% of salary up to a $20,000 maximum benefit per month. Benefits are paid after a 90 day Elimination Period and are payable until your Social Security Retirement Age.
Class 3 (Staff and other Employees): Benefits are 60% of salary up to a $6,000 maximum benefit per month. Benefits are paid after a 90 day Elimination Period and are payable until your Social Security Retirement Age.
