UHC Voluntary Vision Enrollment Process
Enrollment into this plan is Voluntary and uses a paper enrollment form process. The enrollment form is located in the Quick Links toolbar. Please complete Section A but do not indicate a PCP or Dentists since the form is for Vision only. Complete Section C if you want to add dependent coverage. In Section D \’Product Selection\’ only select Vision. Skip Sections E-F and sign the form on the last page where indicated. You do not need an ID card to access your benefits, Participating Providers should be able to confirm coverage by giving them your information at the time services are rendered. You may also easily print an ID card at www.myuhcvision.com under My Benefits simply click on \’Click here to print a Vision ID Card.
Please submit your completed enrollment form to the HR department ASAP. Your enrollment must be completed within the first 30 days of employment.
